Community Support Staff

(Requirements: Must be 18 years of age, Valid Driver’s License, and High School Diploma/GED)

  • Supervise individuals to ensure their health and safety.
  • Perform direct care and supervision, including hand-over-hand assistance, to individuals. Assist and encourage individuals to be as independent as possible in all areas of their lives. Maintain a home-like environment and ensure the personalized attention required by each individual.
  • Train individuals in Activities of Daily Living (ADLs). Example of ADLs include: personal hygiene and grooming, assisting clients with incontinence, household cleaning, health and nutrition, social skills, menu and meal preparation, cooking, shopping, community awareness and interaction, recreation and socialization. Making sure individuals have a presentable appearance according to weather and client activity.
  • Ability to exercise good judgment and remain calm in crisis situations a must.

Support Managers

(Requirements: Must be at least 21 years of age, 3 years’ of experience providing services to adults with disabilities, 2 years’ of experience in a supervisory role, Valid Driver’s license, and Minimum High School Diploma/GED, BA/BS or equivalent experience preferred)

  • Supervise individuals to ensure their health and safety.
  • Perform direct care and supervision, including hand-over-hand assistance, to individuals. Assist and encourage individuals to be as independent as possible in all areas of their lives. Maintain a home-like environment and ensure the personalized attention required by each individual.
  • Train individuals in Activities of Daily Living (ADLs). Example of ADLs include: personal hygiene and grooming, assisting clients with incontinence, household cleaning, health and nutrition, social skills, menu and meal preparation, cooking, shopping, community awareness and interaction, recreation and socialization. Making sure individuals have a presentable appearance according to weather and client activity.
  • Ability to exercise good judgment and remain calm in crisis situations a must.
  • Ensure that the program premises and vehicles are in compliance with State Licensing Standards.
  • Monitor and endorse daily logs, training plans, and other documentation.
  • Monitor all consumer finances in accordance with agency policies.
  • Preparation of the weekly staff schedule and ensuring adequate program coverage in obtained weekly.
  • Participate in program on-call rotation and respond to emergencies, as needed.
  • Supervise subordinate staff to function to meet the needs of the individuals. Train staff on their primary job functions and provide feedback. Foster teambuilding with program participants, colleagues, community   associates, and DDD staff. Develop and implement a teambuilding model with subordinate staff members.
  • Coordinate development, implementation, and revision of IHP’s for all consumers, utilizing the principles of Person Centered Planning.

Assistant Support Managers

(Requirements: Must be at least 21 years of age, 2 years’ of experience providing services to adults with disabilities, Valid Driver’s license, and High School Diploma/GED)

  • Participate in Program On-Call rotation and respond to emergencies, as needed.
  • Ensuring all consumers’ medical files are up to date and meet DDD licensing standards.
  • Create and maintain medical transportation driving direction book.
  • Coordinate weekly food shopping schedule and practices; ensure proper documentation and practices related to weekly menus, diet plans, and food shopping budget and receipts.
  • Supervise subordinate staff to function to meet the needs of the individuals.
  • Assist the Residential Manager to ensure adequate program coverage weekly.
  • Foster teambuilding with program participants, colleagues, community associates, and DDD staff. Develop and implement a teambuilding model with subordinates.
